Step 1: Complete the work order to tell me the basics about what happened to your drive (if you know), what need done (such as file recovery or wiping), and then mail this form, your broken drive, your new drive (unless you want the information put on DVD), and the return envelope or box to the following address:

**NOTE: The US Postal Service has flat-rate boxes and envelopes that are usually more than adequate to mail drives.
**NOTE 2: It's a good idea for you to mail your drive in an anti-static wrapping, which is generally available at any office supply store, computer store, UPS store, etc. It's optional, but will help protect whatever data you have left on the old drive.
**NOTE 3: If possible, try to use padding of some sort. I've seen the way packages are sometimes treated and it's not pretty.
Step 2: Check your email or watch your phone for an invoice from me, to include any discounts. I will also provide you with a report detailing what files I recovered.
Step 3: Payment should be made in the form of a check, money order, or PayPal. As soon as payment has cleared, I will mail your items back to you in the envelope or box you provided in step 1. PayPal is the best option if you want to keep your identity secret since the money only goes from email address to email addres. Payment can be made via paypal to
Step 4: Get a back-up hard drive or an online service like Carbonite.com so you don't have this problem in the future. Obviously, this will limit my future business, but your photos and documents can be priceless to you.
